I've been browsing the "Rules of the Game" rulebook and referee handbook... Information I found is that a referee must take down the players name, number and club. this is then to be submitted by the appropriate action. My interpretation is that as long as there is a last name and club, the player can be found. Unless he has given a wrong name or there is more than one player with that surname. Personally, not giving your full name when cautioned/sent off is rather childish in my eyes. Hopefully an qualified ref can answer this for you Dangerous, Thank you for your time. Having spoken to the ref he admitted no foul language or behaviour was used, he was just following the letter of the law as our player had not stated his full name.
